Some children and young people with physical medical difficulties may require individual programmes to ensure they have full access to school and the curriculum.
Adult support for toileting as specified in a toileting programme, refer to policy document B5c in SEN handbook.
For access to specialist equipment for toileting refer to policy document A 15 in SEN handbook.
A child with a physical difficulty should have access to appropriate hygiene facilities. Refer to building adaptations in SEN handbook
Intimate Care Plan
Children and young people with motor co-ordination difficulties and limited independent self- care skills may require support to manage their intimate care. Schools should complete 'Intimate Care Plans' when support is required to meet these needs.
Fatigue Management Plans
Many children and young people with physical/medical needs experience increased levels of fatigue due to their specific difficulties. Fatigue is more than physical tiredness it can impact on all areas of functioning, including cognitive, communicative etc. Schools should complete a Fatigue Management Plan to ensure the child or young person has the correct strategies in place to recognise and support this area of need.
Positive Behaviour Management Plans
Some children and young people may present with behaviour difficulties. It may then be necessary to implement a behaviour management plan in order to manage and change the behaviours. The school behaviour policy and the school physical intervention policy will guide and inform this process. Consideration will be given to physical or medical needs and, in consultation with parents, a plan will be drawn up to include a risk assessment, with strategies to manage and change the behaviour. The plan may include guidance on the use of physical intervention as an agreed strategy for reducing an identified risk.

Moving and Handling Risk Assessment
Many children and young people with physical and medical needs experience difficulty with independent movement and transitions within the school environment and on off site visits. Such children and young people must have individual risk assessments completed identifying risks and strategies to minimise them. These assessments should also identify any increased risks in practical lessons.
Some children and young people may require specialised assessments carried out by a qualified Moving and Handling Assessor or Occupational Therapist. These need to be re-evaluated on a 6 monthly basis and updated if required.
When writing IEP targets for a child or young person with a statement for Physical/Medical difficulties it is vital to set targets that meet the objectives of the statement. If there are no cognitive/learning difficulties then there should be no cognitive/learning targets.
