Our easy to use wellbeing planning tool helps you take control by building a personalised plan to help you make positive changes to improve your health and wellbeing.
It allows you to set personal goals and identifies independent sources of support locally and nationally to help you achieve them. There are eight key areas to think about which are:

In each of these sections you will find a set of goals to choose from to help you make those positive changes. It takes around 20 minutes to complete and once you have set your goals and selected the information you need, you will be able to download your completed wellbeing plan that you can then use to monitor and review your own progress and perhaps set new goals.
Please note - your plan needs to be created and saved on Cumbria County Council server for downloading purposes only, it will not be shared with anyone and any information you provide will be treated as confidential. This is a self-serve tool and will not result in any intervention by Cumbria County Council. The information cannot be used for any purpose other than originally intended therefore will not be accessed by Cumbria County Council staff.
