Are You Eligible for Adult Social Care?

Referral Process for Transition Cases within Health Care & Community Services (Adult Social Care): 

Adult Social Care have launched a change in their referral process for young people who are approaching year 9 (age 14) who require support from the Transition Social Worker.  From 1st February 2018, all individuals, their families and agencies wishing to refer a young person into Adult Services, Single Point of Access (SPA) should call the appropriate number below for the relevant area to make a referral.

On making a referral, the SPA Officer will request contact and some other basic details of the young person to inform the transition process. Following the allocation of a transition worker, contact will be made in a timely way with the referrer, young person and their representatives to commence the transition process or to provide advice and information.
